Open a new item dialog from a custom form using C#

I have the following code (got it from web) to try to create a new item from for another list. I need the new form open in dialog mode so it will redirect users back to the original form when the new form is closed. I am not sure how to set the reference to the JavaScript files and if the code is correct. Please advise. Thanks a bunch.
