User permissions disappearing from folder

We have an issue where a user keeps losing access to a folder. It appears to happen when a workflow starts on a document. She creates a document and is then unable to access the folder. When checking the status of the workflow it says 'Cancelled' - this would be due to the fact that her permissions have been removed.

Any ideas?
