Get windows form data from sharepoint page and use if statement

Hallo, I've a Windows form test with 12 questions, and I 've embed in a SharePoint page. I'd like that  if the user press "submit" and  answer correctly  10 questions, he can watch a office  365 video  below.

I need to get windows forms data from a SharePoint page.

Which is the best solution?

- Should I use scripr editor in  classical experience, if it's how can I get forms data?

or

- I've installed  SPFX Web part , but doesn't appear the "modern script editor".....

What should I do?
