I am new to Sharepoint and using SharePoint 2013. We have an approval workflow we have created for Change Requests. When the workflow starts, it "Start a Task Process". The individual approves the request and then it goes to the next approver or if rejected goes to rejected. The workflow works perfect, it is the History logging that I am having an issue with. The Task Outcome will not log to the Outcome in the History. I am also trying to log the individual that is approving the request either in the Outcome or Description, but it is not working. I have tried just about every combination that I can think of and still cannot get anything in Outcome or Description to work properly. Any help would be appreciated.
