Creating Dynamic New Forms : Fields shown dependent on YES/NO choice field

HI all,

I'm building a new form to allow people to book resources.

The resources are listed in a drop down field. This covers 95% of the usual traffic.

There are a few odd requests that aren't listed in the drop down field.

To capture these requests I have created 2 other fields 'new resource name' and ' new costpool' fields. 

The idea is that if the resource required is not in the drop down list then they can manually populate the 'new resource name' and ' new costpool' fields.

I have also created a YES/NO choice field called 'Manual input?' Default setting is no.

What I'd like to do is to have the 'new resource name' and ' new costpool' fields hidden in the form and only shown if the manual input field is 'ticked'?

Any help would be greatly appreciated.
