Create a complex survey with SharePoint 2013

Hello there,

I have been trying to design a survey with SharePoint 2013. The "Survey" application is good but not good enough for what I am trying to achieve.
Indeed, I would like to provide the end-users with the ability to fill out some answers with array.

Moreover, I will most definitely need to insert pictures within the body of my questions.

As far as I went, it is not possible to edit such things through SharePoint 2013 is it?

I've digged in and heard that this should be doable through InfoPath Designer 2013 or SharePoint Designer 2013, is it true?
If it is indeed possible, what shall I set up in order to do so?
