Creating a Custom NewForm.aspx

Hi

I have created a list and would like to add some Headings to the NewForm.aspx, so  I created a new form and set as default using SharePoint designer and inserted some code for the heading.  If I then add some more columns to my list they do not display on the form.  Yet when I revert back to using the original NewForm.aspx my new columns are showing.

So I then tried copying the original NewForm.aspx which does work and brings in my columns, however I am not able to add any more headings using code (in between the columns) because when I view the aspx page it does not show me where my columns sit on the form.

I hope someone can point me in the right direction as to where I am going wrong.
